SUMMARY

This position is directly responsible for carrying out the required duties of the Behavioral Health Home Plus (BHHP) program. The BHHP program provides integrated behavioral health and physical health care to identified Blended Case Management consumers in Elk County. Collaborates with internal and external stakeholders, ensures that services delivered meet agency expectations and fulfill contract agreement.

POSITION QUALIFICATIONS

Education

An Associate's Degree in Nursing.

Experience

Five years' experience, Home Health preferred.

Certifications & Licenses

Licensed to practice as an LPN in the Commonwealth of Pennsylvania.

Valid driver's license.
